{"id":18955,"date":"2022-12-05T14:18:59","date_gmt":"2022-12-05T12:18:59","guid":{"rendered":"https:\/\/neuronup.com\/?p=18955"},"modified":"2026-06-25T16:18:08","modified_gmt":"2026-06-25T14:18:08","slug":"traumatisme-cranien-et-neuroreeducation-des-fonctions-executives","status":"publish","type":"post","link":"https:\/\/neuronup.com\/fr\/actualites-de-la-stimulation-cognitive\/lesions-cerebrales\/traumatisme-cranien-et-neuroreeducation-des-fonctions-executives\/","title":{"rendered":"Traumatisme cr\u00e2nio-enc\u00e9phalique et sa r\u00e9\u00e9ducation neuropsychologique des fonctions ex\u00e9cutives"},"content":{"rendered":"\n<p class=\"has-xl-font-size wp-block-paragraph\">La especialista en neuropsicolog\u00eda Mar\u00eda Teresa explica en este art\u00edculo <strong>ce qu&rsquo;est le traumatisme cr\u00e2nio-enc\u00e9phalique et sa r\u00e9\u00e9ducation neuropsychologique<\/strong> en funciones ejecutivas.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">\u00bfQu\u00e9 son los Traumas Craneoencef\u00e1licos (TCC)?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Se define como una <strong>alt\u00e9ration du fonctionnement<\/strong> <strong>c\u00e9r\u00e9bral<\/strong>, <strong>caus\u00e9e par une force externe<\/strong> (Menon, Schwab, Wright y Maas, 2010).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les traumatismes cr\u00e2nio-enc\u00e9phalique (TCC) sont un probl\u00e8me critique de sant\u00e9 publique, tant par leurs taux \u00e9lev\u00e9s de mortalit\u00e9 que par les incapacit\u00e9s pr\u00e9sent\u00e9es par les patients qui y survivent, manifestant des difficult\u00e9s au niveau :<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cognitif, \u00e9motionnel, familial, social et professionnel.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Lesquelles affectent leur qualit\u00e9 de vie (Arango-Lasprilla, Quijano y Cuervo, 2010; Corrigan, Selassie y Orman, 2010; Garc\u00eda-Rudolph y Gibert, 2015; Park et al., 2015; Santana et al., 2015). <br><br>En neuropsychologie, la conception de programmes de r\u00e9\u00e9ducation se r\u00e9alise depuis l&rsquo;<strong>approche cognitive<\/strong>, car on consid\u00e8re qu&rsquo;am\u00e9liorer la capacit\u00e9 mentale des patients atteints d&rsquo;un&nbsp;<strong>traumatisme cr\u00e2nio-enc\u00e9phalique<\/strong> a un effet direct sur leur fonctionnalit\u00e9.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Tipos de traumatismo craneoencef\u00e1lico (TCC):<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Les TCC ouverts<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les traumatismes cr\u00e2nio-enc\u00e9phalique ouverts ocurren cuando se produce una<strong> fracture ou perforation de<\/strong> <strong>la vo\u00fbte cr\u00e2nienne<\/strong>, produciendo una<strong> plaie dans<\/strong> <strong>le tissu c\u00e9r\u00e9bral<\/strong> y exponiendo o dejando en contacto con el aire la masa encef\u00e1lica.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Les TCC ferm\u00e9s<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les traumatismes cr\u00e2nio-enc\u00e9phalique ferm\u00e9s solo <strong>affectent le tissu c\u00e9r\u00e9bral <\/strong>(Le\u00f3n-Carri\u00f3n, 1995).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les deux types de traumatismes pr\u00e9sentent g\u00e9n\u00e9ralement une atteinte focale et une autre diffuse, en raison de l&rsquo;impact re\u00e7u. La premi\u00e8re correspond \u00e0 la l\u00e9sion g\u00e9n\u00e9r\u00e9e \u00e0 l&rsquo;endroit du cerveau ayant re\u00e7u l&rsquo;impact. La seconde est celle qui n&rsquo;occupe pas un volume bien d\u00e9fini \u00e0 l&rsquo;int\u00e9rieur du compartiment intracr\u00e2nien, mais qui engendre, tout comme la l\u00e9sion focale, des s\u00e9quelles neurologiques (Gonz\u00e1lez, Pueyo y Serra, 2004). <br><br>Habituellement, les l\u00e9sions focales se caract\u00e9risent par des alt\u00e9rations du fonctionnement du lobe frontal et temporal, car ce sont les zones les plus susceptibles dans les TCC ferm\u00e9s; dans les ouverts, cela d\u00e9pendra du lieu o\u00f9 l&rsquo;os du cr\u00e2ne peut \u00eatre affect\u00e9. Mu\u00f1ana-Rodr\u00edguez y Ram\u00edrez-El\u00edas, 2013; Santa Cruz y Herrera, 2006; Poca, 2006).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">La intervenci\u00f3n tras sufrir un TC<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">La intervenci\u00f3n posterior a sufrir un TCC suele incluir <strong>r\u00e9\u00e9ducation physique et cognitive<\/strong>. En esta \u00faltima es conveniente que est\u00e9 dirigida a las funciones cognitivas de alto procesamiento, como lo son las funciones ejecutivas, pues suelen ser una de las que m\u00e1s se afectan, tanto en los da\u00f1os focales como difusos, generados por el TCC (Garc\u00eda-Molina, Ense\u00f1at-Cantallops, S\u00e1nchez-Carri\u00f3n, Tormos y Roig-Rovira, 2014).<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Las funciones ejecutivas en persona con traumatismo craneoencef\u00e1lico<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">El concepto de funcionamiento ejecutivo hace referencia a un conjunto de operaciones cognitivas de alto nivel como la planificaci\u00f3n, la toma de decisiones, la flexibilidad, entre otras, las cuales controlan y regulan el comportamiento, dirigi\u00e9ndolo hacia una meta, formando sus objetivos y planeando c\u00f3mo se pueden llevar a cabo.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Estas mismas funciones tambi\u00e9n son reconocidas como capacidades mentales esenciales, para llevar a cabo una conducta creativa y aceptada socialmente. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Adem\u00e1s, las funciones ejecutivas se van a ir volvi\u00e9ndose m\u00e1s complejas a lo largo del desarrollo; algunas de ellas tienen una aparici\u00f3n temprana, lo que posibilita la aparici\u00f3n y complejizaci\u00f3n de otras funciones ejecutivas (Bomb\u00edn-Gonz\u00e1lez et al., 2014; Tirapu-Ust\u00e1rrez, Garc\u00eda-Molina, Luna-Lario, Verdejo-Garc\u00eda y Rios-Lago, 2012).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Para Tirapu-Ust\u00e1rrez et al. (2017), la revisi\u00f3n sistem\u00e1tica de an\u00e1lisis factoriales de las funciones ejecutivas result\u00f3 en una propuesta integradora de los procesos de control ejecutivo, tales como:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>La velocidad de procesamiento<\/strong>: cantidad de informaci\u00f3n que puede ser procesada por unidad de tiempo o la velocidad para realizar operaciones cognitivas;<\/li>\n\n\n\n<li><strong>la memoria de trabajo<\/strong>: capacidad para registrar, codificar, mantener y manipular informaci\u00f3n en l\u00ednea;<\/li>\n\n\n\n<li><strong>la fluidez verbal<\/strong>: capacidad de acceso a la recuperaci\u00f3n de informaci\u00f3n, de la memoria sem\u00e1ntica y activaci\u00f3n para la b\u00fasqueda de palabras;<\/li>\n\n\n\n<li><strong>la inhibici\u00f3n<\/strong>: control de la interferencia y distractores o atenci\u00f3n selectiva,<\/li>\n\n\n\n<li><strong>la ejecuci\u00f3n dual<\/strong>: capacidad para prestar atenci\u00f3n simult\u00e1neamente a varios est\u00edmulos,<\/li>\n\n\n\n<li><strong>la flexibilidad cognitiva<\/strong>: alternancia,<\/li>\n\n\n\n<li><strong>la planificaci\u00f3n<\/strong>: monitorizaci\u00f3n y control de la conducta,<\/li>\n\n\n\n<li><strong>la toma de decisiones<\/strong>: papel de las emociones en el razonamiento.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>La funci\u00f3n ejecutiva juega un papel primordial en la vida de los seres humanos<\/strong>, dado que se trata de un conjunto de procesos cognitivos con distintos componentes independientes, pero con \u00edntimas relaciones entre s\u00ed para controlar y modular el comportamiento. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Una vez estas funciones se ven afectadas por un da\u00f1o neurol\u00f3gico, como en un traumatismo cr\u00e2nio-enc\u00e9phalique, las deficiencias ejecutivas generan una multiplicidad de manifestaciones cognitivas, comportamentales y emocionales, las cuales interfieren en el adecuado desenvolvimiento de la persona en su vida cotidiana, creando dificultades para recuperar una vida normal y productiva.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Rehabilitaci\u00f3n neuropsicol\u00f3gica en pacientes con TCC<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La rehabilitaci\u00f3n puede definirse como una <strong>aplicaci\u00f3n sistem\u00e1tica de actividades terap\u00e9uticas<\/strong>, <strong>orientadas a mejorar la funcionalidad del paciente<\/strong>, a partir de la comprensi\u00f3n de sus d\u00e9ficits (Cicerone et al. como se cita en van Heugten, Greg\u00f3rio y Wade, 2012). <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La intervenci\u00f3n debe tener <strong>validez ecol\u00f3gica<\/strong>, para que tenga un impacto real en la vida diaria del paciente, con el objetivo de que pueda extrapolar y generalizar en su cotidianidad, lo aprendido en consulta (Carvajal-Castrill\u00f3n y Restrepo, 2013).<\/p>\n\n<h3 class=\"wp-block-heading\">Programmes d&rsquo;\u00e9valuation et de r\u00e9habilitation personnalis\u00e9s<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les propositions de la r\u00e9habilitation cognitive, depuis la neuropsychologie contemporaine, sugg\u00e8rent l&rsquo;\u00e9laboration de programmes d&rsquo;\u00e9valuation et de r\u00e9habilitation <strong>individualis\u00e9s <\/strong>pour chaque pathologie, avec des attentes et des objectifs clairs et partag\u00e9s pour le patient et sa famille (Calder\u00f3n, Cadavid-Ruiz et Santos, 2016; Carvajal-Castrill\u00f3n et Restrepo, 2013; R\u00edos, Mu\u00f1oz et Pa\u00fal-Lapedriza, 2007; Tate, Aird et Taylor, 2013). <\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Leurs programmes de r\u00e9habilitation consistent en <strong>t\u00e2ches organis\u00e9es hi\u00e9rarchiquement par niveau de difficult\u00e9 <\/strong>et qui n\u00e9cessitent l&rsquo;utilisation r\u00e9p\u00e9t\u00e9e des fonctions d\u00e9t\u00e9rior\u00e9es. Ces programmes pr\u00e9cisent que le degr\u00e9 de r\u00e9cup\u00e9ration fonctionnelle du patient d\u00e9pendra du nombre de r\u00e9p\u00e9titions et du type de t\u00e2che r\u00e9alis\u00e9e durant le traitement (Garc\u00eda-Rudolph et Gibert, 2015).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">En neuropsychologie, la conception des programmes de r\u00e9habilitation se fait depuis l&rsquo;<strong>approche cognitive<\/strong>, car on consid\u00e8re qu&rsquo;am\u00e9liorer la capacit\u00e9 mentale des patients a un effet direct sur leur fonctionnalit\u00e9. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">De plus, ces programmes soulignent l&rsquo;importance d&rsquo;<strong>ajuster les programmes aux besoins individuels <\/strong>du patient \u00e0 partir de techniques restauratrices ou compensatoires. La premi\u00e8re renvoie au renforcement, au renforcement ou \u00e0 la restauration des processus cognitifs d\u00e9t\u00e9rior\u00e9s; la seconde pr\u00e9sente des moyens pour compenser la fonction alt\u00e9r\u00e9e, par le biais de l&rsquo;utilisation de ressources externes au patient, par exemple des rappels ou des alarmes, entre autres (Barman et al., 2016; Evald, 2015; Tsaousides, D\u2019Antonio, Varbanova et Spielman, 2014).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Or, la r\u00e9habilitation cognitive doit garder \u00e0 l&rsquo;esprit que le traumatisme cr\u00e2nio-c\u00e9r\u00e9bral est une condition m\u00e9dicale qui concerne divers domaines de la sant\u00e9; elle requiert de :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Une prise en charge neurologique pour moduler et superviser les l\u00e9sions g\u00e9n\u00e9r\u00e9es sur le tissu c\u00e9r\u00e9bral,<\/li>\n\n\n\n<li>une intervention neuropsychologique pour r\u00e9cup\u00e9rer le plus haut degr\u00e9 de fonctionnalit\u00e9 possible chez le patient,<\/li>\n\n\n\n<li>une gestion sociale pour soutenir la fonctionnalit\u00e9 du patient dans les contextes quotidiens o\u00f9 il peut \u00e9voluer.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Les r\u00e9sultats des recherches<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Les r\u00e9sultats des recherches sur la r\u00e9habilitation chez les patients atteints de traumatisme cr\u00e2nio-c\u00e9r\u00e9bral indiquent que <strong>les meilleurs r\u00e9sultats sont obtenus lorsque les programmes d&rsquo;intervention visent une prise en charge globale et interdisciplinaire <\/strong>de la condition m\u00e9dicale et psychosociale v\u00e9cue par le patient, ce qui inclut l&rsquo;intervention sur son plan cognitif, \u00e9motionnel, familial et social. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ces initiatives ne doivent pas seulement viser la r\u00e9habilitation du patient avec TCC, mais doivent \u00e9galement s&rsquo;orienter vers la promotion de sa sant\u00e9, ce qui suppose la mise en \u0153uvre de mesures pour adopter des modes de vie sains.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Referencias<\/h2>\n\n\n\n<p class=\"has-sm-font-size wp-block-paragraph\">Arango-Lasprilla, J.C., Quijano, M.C. et Cuervo, M.T.
