Heute wird es als <strong>dynamischer und fortschreitender Prozess<\/strong> verstanden und nicht als isolierte Verletzung, mit kurz-, mittel- und langfristigen Auswirkungen.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Es handelt sich um ein <strong>globales Gesundheitsproblem<\/strong> mit j\u00e4hrlich Millionen von F\u00e4llen und einer hohen Belastung durch Behinderungen, von denen insbesondere <strong>\u00e4ltere Menschen (St\u00fcrze)<\/strong> und <strong>junge Erwachsene (Verkehrsunf\u00e4lle)<\/strong> betroffen sind.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Pathophysiologie: ein Prozess in Phasen<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Das SHT entwickelt sich in verschiedenen miteinander verbundenen Phasen:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Prim\u00e4rverletzung:<\/strong> unmittelbare und irreversible Sch\u00e4digung (Kontusionen, H\u00e4matome, diffuse axonale Sch\u00e4digung).<\/li>\n\n\n\n<li><strong>Sekund\u00e4rverletzung:<\/strong> neurochemische Kaskade (\u00d6dem, Entz\u00fcndung, Exzitotoxizit\u00e4t, Hypoxie), die die initiale Sch\u00e4digung verschlimmert.<\/li>\n\n\n\n<li><strong>Terti\u00e4rverletzung:<\/strong> verz\u00f6gert auftretende Prozesse wie Neurodegeneration und programmierter Zelltod.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Ein zentraler Begriff ist die <strong>verz\u00f6gerte Verschlechterung<\/strong>: Scheinbar stabile Patientinnen und Patienten k\u00f6nnen sich Stunden sp\u00e4ter durch die <strong>Ausweitung von H\u00e4matomen oder die Zunahme des \u00d6dems<\/strong> verschlechtern.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Intrakranieller Druck und zerebrale Perfusion<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Die <strong>Monro-Kellie-Doktrin<\/strong> besagt, dass der Sch\u00e4del ein geschlossener Raum ist, in dem Gehirn, Blut und Liquor koexistieren. Wenn das Volumen eines Bestandteils zunimmt, m\u00fcssen die anderen dies kompensieren.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Die entscheidende Gr\u00f6\u00dfe ist der <strong>zerebrale Perfusionsdruck (CPP)<\/strong>:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>CPP = mittlerer arterieller Druck \u2013 intrakranieller Druck<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ein Anstieg des intrakraniellen Drucks kann eine <strong>zerebrale Isch\u00e4mie und Herniationen<\/strong> ausl\u00f6sen \u2013 potenziell lebensbedrohliche Zust\u00e4nde.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Klinische Beurteilung: mehr als die Glasgow Coma Scale<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Die <strong>Glasgow Coma Scale (GCS)<\/strong> ist weiterhin grundlegend f\u00fcr die Einstufung des SHT:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Leicht:<\/strong> 13\u201315<\/li>\n\n\n\n<li><strong>Mittelgradig:<\/strong> 9\u201312<\/li>\n\n\n\n<li><strong>Schwer:<\/strong> \u22648<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Heute ist jedoch anerkannt, dass sie <strong>allein nicht ausreicht<\/strong>. Der moderne Ansatz ber\u00fccksichtigt zus\u00e4tzlich:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Serumbiomarker<\/strong><\/li>\n\n\n\n<li><strong>Fortgeschrittene Neurobildgebung<\/strong><\/li>\n\n\n\n<li><strong>Klinische und kontextbezogene Faktoren<\/strong><\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Diagnostik: Bildgebung und Biomarker<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Die <strong>Computertomografie (CT)<\/strong> ist die entscheidende initiale Untersuchung zum Nachweis schwerer Verletzungen.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Die <strong>Magnetresonanztomografie (MRT)<\/strong> erm\u00f6glicht den Nachweis subtilerer Verletzungen, insbesondere einer <strong>diffusen axonalen Sch\u00e4digung<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In den vergangenen Jahren wurden Biomarker wie die folgenden entwickelt:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>GFAP:<\/strong> weist auf eine astrogliale Sch\u00e4digung hin<\/li>\n\n\n\n<li><strong>UCH-L1:<\/strong> weist auf eine neuronale Sch\u00e4digung hin<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Sie erm\u00f6glichen ein <strong>verbessertes Screening und den Nachweis von Verletzungen sogar bei unauff\u00e4lliger CT<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">K\u00fcnstliche Intelligenz und fortgeschrittene Diagnostik<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Die <strong>k\u00fcnstliche Intelligenz (KI)<\/strong> ver\u00e4ndert die Beurteilung des SHT durch:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Automatisierte Bildanalyse<\/strong><\/li>\n\n\n\n<li><strong>Vorhersage des klinischen Verlaufs<\/strong><\/li>\n\n\n\n<li><strong>Fr\u00fcherkennung von Komplikationen<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Dennoch <strong>ersetzt sie nicht das klinische Urteil<\/strong> und muss in unterschiedlichen Kontexten validiert werden.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Neurokritische Behandlung<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Das wichtigste Ziel besteht darin, <strong>eine Sekund\u00e4rverletzung zu verhindern<\/strong> und die zerebrale Perfusion aufrechtzuerhalten.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Zu den zentralen Strategien geh\u00f6ren:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Kontrolle des intrakraniellen Drucks<\/strong><\/li>\n\n\n\n<li><strong>Multimodales Neuromonitoring<\/strong><\/li>\n\n\n\n<li><strong>Osmotherapie:<\/strong> Mannitol oder hypertonische Kochsalzl\u00f6sung<\/li>\n\n\n\n<li><strong>Externe Ventrikeldrainage<\/strong><\/li>\n\n\n\n<li><strong>Dekompressive Kraniektomie<\/strong> bei therapierefrakt\u00e4ren F\u00e4llen<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Eine <strong>prophylaktische Hyperventilation<\/strong> wird wegen des Isch\u00e4mierisikos nicht mehr routinem\u00e4\u00dfig empfohlen.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Kognitive und Verhaltensfolgen<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Das \u00dcberstehen der Akutphase bedeutet keine vollst\u00e4ndige Genesung.
