{"id":33254,"date":"2024-10-08T23:14:00","date_gmt":"2024-10-08T21:14:00","guid":{"rendered":"https:\/\/neuronup.com\/?p=33254"},"modified":"2024-10-08T23:14:00","modified_gmt":"2024-10-08T21:14:00","slug":"estimulacao-tdcs-na-reabilitacao-da-heminegligencia-apos-um-avc","status":"publish","type":"post","link":"https:\/\/neuronup.com\/br\/noticias-de-estimulacao-cognitiva\/dano-cerebral\/avc\/estimulacao-tdcs-na-reabilitacao-da-heminegligencia-apos-um-avc\/","title":{"rendered":"A reabilita\u00e7\u00e3o da hemineglig\u00eancia ap\u00f3s o AVC"},"content":{"rendered":"\n<p class=\"has-xl-font-size wp-block-paragraph\">Os neurologistas Bego\u00f1a Gonz\u00e1lez, Elena Mu\u00f1oz e Juan Pablo Romero nos apresentam a <strong>Estimula\u00e7\u00e3o transcraniana por corrente cont\u00ednua (tDCS)<\/strong> como m\u00e9todo para <strong>a reabilita\u00e7\u00e3o da hemineglig\u00eancia<\/strong> ap\u00f3s o <strong>AVC<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Defini\u00e7\u00e3o de hemineglig\u00eancia e repercuss\u00f5es funcionais<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A <strong>hemineglig\u00eancia<\/strong> implica uma <strong>dificuldade para atender ao hemicampo contralateral<\/strong> (habitualmente o esquerdo) \u00e0 \u00e1rea cerebral lesionada, e pode abarcar desde uma omiss\u00e3o de objetos que se localizem no hemicampo esquerdo do espa\u00e7o at\u00e9 a falta de aten\u00e7\u00e3o, reconhecimento e at\u00e9 nega\u00e7\u00e3o de partes do pr\u00f3prio corpo. A n\u00edvel funcional, a hemineglig\u00eancia d\u00e1 lugar a <strong>dificuldades severas na independ\u00eancia da pessoa afetada<\/strong>, tanto em atividades b\u00e1sicas da vida di\u00e1ria (higiene, vestir-se, alimenta\u00e7\u00e3o, etc.) como instrumentais (manuseio de dinheiro, prepara\u00e7\u00e3o de comida, transporte p\u00fablico ou deambula\u00e7\u00e3o independente).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Al\u00e9m disso, a presen\u00e7a de hemineglig\u00eancia tem sido associada a tempos de internamento hospitalar e de reabilita\u00e7\u00e3o mais prolongados, maior risco de quedas e uma fraca recupera\u00e7\u00e3o motora e funcional (Chen et al., 2015; Jehkonen et al., 2006; Wilkinson et al., 2012).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Recupera\u00e7\u00e3o da hemineglig\u00eancia<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Quanto ao progn\u00f3stico de recupera\u00e7\u00e3o da hemineglig\u00eancia, em estudos realizados entre os 8 e os 12 meses posteriores a um acidente vascular cerebral com presen\u00e7a de hemineglig\u00eancia, encontraram que essa altera\u00e7\u00e3o persistia em um ter\u00e7o dos casos (Colombo et&nbsp;al., 1982; Karnath et&nbsp;al., 2011) ou ocorria uma escassa recupera\u00e7\u00e3o da mesma (Kalra et&nbsp;al., 1997; Katz et&nbsp;al., 1999; Luukkainen-Markkula et&nbsp;al., 2014; Paolucci et&nbsp;al., 2001).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Terapias atuais<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Hoje em dia, as abordagens terap\u00eauticas dispon\u00edveis para a <strong>reabilita\u00e7\u00e3o da hemineglig\u00eancia<\/strong>, como por exemplo a adapta\u00e7\u00e3o prism\u00e1tica, o treino visuoespacial, a terapia de imagens mentais ou a estimula\u00e7\u00e3o optocin\u00e9tica, t\u00eam uma <strong>efic\u00e1cia cl\u00ednica limitada<\/strong> e, por vezes, pouco duradoura (Azouvi et al., 2017; Fasotti &amp; van Kessel, 2013). Devido \u00e0 persist\u00eancia da sintomatologia e ao impacto que tem na independ\u00eancia do paciente, \u00e9 essencial desenvolver novos m\u00e9todos de tratamento e abordar as disfun\u00e7\u00f5es cerebrais subjacentes de forma adequada. Tamb\u00e9m participam no projeto o doutor Marcos R\u00edos-Lago, coordenador da unidade de dano cerebral do Hospital Beata Mar\u00eda Ana, Bego\u00f1a Gonz\u00e1lez Rodr\u00edguez e David de Nore\u00f1a Mart\u00ednez, neuropsic\u00f3logos da Unidade, e os investigadores pr\u00e9-doutorais da Universidad Francisco de Vitoria, Francisco S\u00e1nchez, Yeray Gonz\u00e1lez e Aida Arroyo.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Objetivo<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">O objetivo deste projeto \u00e9 <strong>validar um protocolo de neuromodula\u00e7\u00e3o direcionado ao tratamento da hemineglig\u00eancia secund\u00e1ria a AVC isqu\u00eamico<\/strong> em pacientes que se encontram em fase subaguda (3 a 12 meses desde a les\u00e3o). Esta t\u00e9cnica busca uma melhoria da sintomatologia mediante a redu\u00e7\u00e3o da hiperatividade patol\u00f3gica que desenvolve o hemisf\u00e9rio n\u00e3o lesado ap\u00f3s um acidente vascular cerebral localizado na art\u00e9ria cerebral m\u00e9dia direita, atrav\u00e9s da aplica\u00e7\u00e3o de um programa de estimula\u00e7\u00e3o tDCS multiss\u00edtio.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">T\u00e9cnicas de neuromodula\u00e7\u00e3o<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A <strong>estimula\u00e7\u00e3o cerebral n\u00e3o invasiva<\/strong> refere-se a diferentes t\u00e9cnicas neurofisiol\u00f3gicas que permitem a modula\u00e7\u00e3o da atividade cerebral de maneira segura e n\u00e3o invasiva (Bikson et&nbsp;al., 2016). Atrav\u00e9s destas t\u00e9cnicas podemos tanto aumentar a excitabilidade do c\u00f3rtex cerebral como diminu\u00ed-la. As t\u00e9cnicas de estimula\u00e7\u00e3o cerebral n\u00e3o invasiva mais utilizadas na atualidade s\u00e3o a <strong>estimula\u00e7\u00e3o magn\u00e9tica transcraniana<\/strong> (TMS) e a <strong>estimula\u00e7\u00e3o transcraniana por corrente cont\u00ednua<\/strong> (tDCS), das quais existe evid\u00eancia emp\u00edrica suficiente que sustenta o seu potencial para modular a atividade cerebral a curto e longo prazo (Hummel &amp; Cohen, 2006).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Estimula\u00e7\u00e3o transcraniana por corrente cont\u00ednua (tDCS)<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A tDCS permite a <strong>modula\u00e7\u00e3o da atividade cortical mediante a aplica\u00e7\u00e3o de uma corrente el\u00e9trica<\/strong> de baixa intensidade (geralmente entre 1 mA e 2 mA) colocando dois ou mais eletrodos sobre o couro cabeludo do paciente. Deste modo, a corrente el\u00e9trica flui entre os eletrodos, desde o \u00e2nodo at\u00e9 o c\u00e1todo, aumentando a excitabilidade cortical abaixo do \u00e2nodo e diminuindo-a sob o c\u00e1todo. Por outro lado, a tDCS multiss\u00edtio, ou de alta defini\u00e7\u00e3o, permite realizar uma estimula\u00e7\u00e3o mais precisa e localizada da \u00e1rea cerebral selecionada.<\/p>\n\n\n<div class=\"wp-block-image image-interface\">\n<figure class=\"aligncenter size-full\"><img decoding=\"async\" src=\"https:\/\/neuronup.com\/wp-content\/uploads\/2024\/09\/Estimulacion-transcraneal-con-corriente-directa-tdcs.webp\" alt=\"Esquema de tDCS: c\u00e9rebro e cr\u00e2nio com eletrodos no couro cabeludo, com etiquetas cr\u00e2nio, corrente el\u00e9trica e c\u00e1todo.\" class=\"wp-image-33300\"\/><figcaption class=\"wp-element-caption\">O que \u00e9 a Estimula\u00e7\u00e3o transcraniana com corrente cont\u00ednua (tDCS)?<\/figcaption><\/figure>\n<\/div>\n\n\n<div style=\"height:25px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Al\u00e9m disso, a tDCS demonstrou ser uma t\u00e9cnica eficaz como abordagem terap\u00eautica complementar aos tratamentos mais convencionais na reabilita\u00e7\u00e3o p\u00f3s-AVC (Edwards &amp; Fregni, 2008), mostrando resultados promissores na <strong>reabilita\u00e7\u00e3o da hemineglig\u00eancia visuoespacial<\/strong> (p. ex., Bang &amp; Bong, 2015; Ladavas et al., 2015; Yi et al., 2016).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Com base no modelo de rivalidade inter-hemisf\u00e9rica de Kinsbourne (Kinsbourne, 1977) e em fun\u00e7\u00e3o dos par\u00e2metros da estimula\u00e7\u00e3o, \u00e9 poss\u00edvel propor diferentes abordagens de estimula\u00e7\u00e3o, como <strong>aumentar a atividade cerebral do hemisf\u00e9rio lesionado, inibir a hiperatividade patol\u00f3gica do hemisf\u00e9rio saud\u00e1vel<\/strong> ou uma combina\u00e7\u00e3o de ambas (Zebhauser et&nbsp;al., 2019).<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Projeto de reabilita\u00e7\u00e3o com tDCS<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Neste projeto aplicamos um protocolo de <strong>tDCS<\/strong> com dura\u00e7\u00e3o de 20 minutos di\u00e1rios durante 10 dias consecutivos (de segunda a sexta), com intensidade de 2 mA. A estimula\u00e7\u00e3o \u00e9 aplicada com o c\u00e1todo posicionado em P3 (seguindo o sistema internacional 10\/20 para a coloca\u00e7\u00e3o de eletrodos de EEG), estando distribu\u00eddos os eletrodos de retorno em C3, CP5, CP1, Pz, PO3, PO7, P7. A facilidade de uso e a ampla adaptabilidade \u00e0s caracter\u00edsticas de cada um dos utilizadores permite desenhar de forma simples e ajustada os programas de interven\u00e7\u00e3o personalizados para cada pessoa.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nove exerc\u00edcios espec\u00edficos dirigidos \u00e0 estimula\u00e7\u00e3o e reabilita\u00e7\u00e3o da hemineglig\u00eancia foram selecionados e combinados ao longo das 10 sess\u00f5es de interven\u00e7\u00e3o (4 tarefas em cada sess\u00e3o), com dura\u00e7\u00e3o de 30 minutos cada uma. A configura\u00e7\u00e3o da plataforma permite programar as sess\u00f5es com o objetivo de aumentar progressivamente a dificuldade das tarefas em fun\u00e7\u00e3o dos progressos alcan\u00e7ados pelo participante. A avalia\u00e7\u00e3o neuropsicol\u00f3gica visa identificar a presen\u00e7a de <strong>hemineglig\u00eancia<\/strong> e qual \u00e9 a gravidade da sintomatologia, avaliando tamb\u00e9m o impacto funcional dessa altera\u00e7\u00e3o no seu funcionamento quotidiano e a altera\u00e7\u00e3o ou preserva\u00e7\u00e3o de outras fun\u00e7\u00f5es cognitivas, como a <strong>aten\u00e7\u00e3o e a mem\u00f3ria de trabalho<\/strong>. A avalia\u00e7\u00e3o neuropsicol\u00f3gica e o EEG ser\u00e3o realizados tr\u00eas dias antes do in\u00edcio da interven\u00e7\u00e3o com <strong>tDCS e NeuronUP<\/strong>. O mesmo protocolo de avalia\u00e7\u00e3o ser\u00e1 repetido tr\u00eas dias ap\u00f3s o fim da interven\u00e7\u00e3o, com o objetivo de analisar as melhorias cognitivas que possam ter ocorrido, as altera\u00e7\u00f5es nas medidas fisiol\u00f3gicas (EEG), e a correla\u00e7\u00e3o entre ambas as medidas.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Conte\u00fado<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Os participantes s\u00e3o atribu\u00eddos a um dos dois grupos de interven\u00e7\u00e3o; o programa de interven\u00e7\u00e3o consiste em 10 sess\u00f5es de reabilita\u00e7\u00e3o neuropsicol\u00f3gica com <strong>NeuronUP<\/strong> ao longo de duas semanas (de segunda a sexta). Um dos grupos (grupo ativo) recebe reabilita\u00e7\u00e3o neuropsicol\u00f3gica durante 30 min combinada com tDCS ativa durante 20 minutos (come\u00e7ando 5 min ap\u00f3s o in\u00edcio do NeuronUP). O outro grupo (grupo placebo) realiza o mesmo programa de reabilita\u00e7\u00e3o neuropsicol\u00f3gica mas combinado com tDCS placebo (denominado sham tDCS) durante o mesmo tempo.
