Neste estudo randomizado, comparou-se o efeito da estimula\u00e7\u00e3o transcraniana por corrente cont\u00ednua (tDCS) aplicada sobre o c\u00f3rtex motor prim\u00e1rio juntamente com treinamento cognitivo (M1 + TC) frente \u00e0 estimula\u00e7\u00e3o sobre o c\u00f3rtex dorsolateral pr\u00e9-frontal combinada com o mesmo treinamento (DLPFC + TC). Sessenta e tr\u00eas pacientes completaram 15 sess\u00f5es do tratamento, com avalia\u00e7\u00f5es neuropsicol\u00f3gicas e cl\u00ednicas realizadas no in\u00edcio, ap\u00f3s a interven\u00e7\u00e3o e um m\u00eas de acompanhamento.<\/p>\n\n\n\n<p class=\"has-sm-font-size\">Os resultados mostraram que o protocolo M1 + TC ofereceu uma ligeira vantagem na redu\u00e7\u00e3o da fadiga e na melhora do sono, enquanto ambos os grupos experimentaram melhoras significativas na cogni\u00e7\u00e3o, ansiedade, depress\u00e3o, dor e qualidade do sono, que se mantiveram ao longo do tempo. Sessenta participantes foram designados aleatoriamente ao programa ou a uma lista de espera e avaliados antes, depois e nove meses ap\u00f3s a interven\u00e7\u00e3o. <\/p>\n\n\n\n<p class=\"has-sm-font-size\">Ap\u00f3s 12 semanas de treinamento, o grupo de interven\u00e7\u00e3o mostrou melhores resultados em mem\u00f3ria verbal imediata, aprendizagem verbal e reconhecimento visual frente ao grupo controle. N\u00e3o foram observadas mudan\u00e7as em mem\u00f3ria visual, percep\u00e7\u00e3o visuoespacial, qualidade de vida nem participa\u00e7\u00e3o. Al\u00e9m disso, as melhoras iniciais n\u00e3o se mantiveram aos nove meses. Este trabalho apresenta um protocolo para implementar um programa de estimula\u00e7\u00e3o cognitiva e emocional orientado a melhorar o desempenho acad\u00eamico e a qualidade de vida de crian\u00e7as neste contexto. <\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">O estudo prop\u00f5e um ensaio cl\u00ednico randomizado de 24 sess\u00f5es utilizando a plataforma digital NeuronUP para trabalhar aten\u00e7\u00e3o, linguagem, fun\u00e7\u00f5es executivas e cogni\u00e7\u00e3o social. Neste estudo, utilizaram-se dados de usu\u00e1rios de programas de estimula\u00e7\u00e3o cognitiva para examinar se o desempenho em distintos materiais de treinamento neuropsicol\u00f3gico podia predizer o decl\u00ednio cognitivo um ano depois.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">Foram analisados 124.610 registros procedentes de 7.902 participantes que completaram 215 testes agrupados em 29 dom\u00ednios cognitivos. Mediante modelos longitudinais e t\u00e9cnicas de aprendizado de m\u00e1quina com valida\u00e7\u00e3o cruzada, observou-se que v\u00e1rios dom\u00ednios cognitivos permitiam predizer o decl\u00ednio futuro com alta precis\u00e3o, enquanto outros mostravam um desempenho mais limitado. Este trabalho descreve o programa ambulatorial implementado no Instituto de Medicina F\u00edsica e Reabilita\u00e7\u00e3o do Hospital das Cl\u00ednicas da Universidade de S\u00e3o Paulo para atender a pacientes com COVID-19 grave ou cr\u00edtico ap\u00f3s sua hospitaliza\u00e7\u00e3o. <br><br>Doze adultos participaram de uma avalia\u00e7\u00e3o integral que incluiu medidas funcionais, cognitivas, emocionais, nutricionais e de for\u00e7a muscular. O programa de reabilita\u00e7\u00e3o combinou estimula\u00e7\u00e3o el\u00e9trica e magn\u00e9tica, exerc\u00edcios isocin\u00e9ticos, tratamento da dor, estimula\u00e7\u00e3o cognitiva, apoio emocional, treinamento ocupacional e educa\u00e7\u00e3o sanit\u00e1ria mediante um aplicativo m\u00f3vel. As sess\u00f5es foram realizadas duas vezes por semana at\u00e9 cumprir crit\u00e9rios de alta.<br><br>Os resultados mostraram melhoras significativas na dor e na mobilidade (VAS e TUG), junto com tend\u00eancias favor\u00e1veis em independ\u00eancia funcional, for\u00e7a de preens\u00e3o, capacidade de marcha, ansiedade e estado funcional p\u00f3s-COVID. Embora os resultados das interven\u00e7\u00f5es cognitivas sejam heterog\u00eaneos, existe evid\u00eancia de que a reabilita\u00e7\u00e3o neuropsicol\u00f3gica assistida por computador pode melhorar o desempenho cognitivo. Este estudo avaliou a efic\u00e1cia de um programa de reabilita\u00e7\u00e3o cognitiva digital em pacientes com EM remitente-recorrente e comprometimento cognitivo leve ou moderado. <\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">Doze pacientes foram avaliados em tr\u00eas momentos (in\u00edcio, semana 8 e semana 16) e designados aleatoriamente a receber tratamento ap\u00f3s a primeira ou a segunda avalia\u00e7\u00e3o. Esta tese teve como objetivo otimizar os efeitos das interven\u00e7\u00f5es cognitivas em crian\u00e7as com paralisia cerebral, mediante tr\u00eas estudos complementares: uma revis\u00e3o sistem\u00e1tica, um estudo transversal e um ensaio cl\u00ednico randomizado.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">A revis\u00e3o mostrou que a maioria das interven\u00e7\u00f5es estudadas s\u00e3o f\u00edsicas, embora algumas melhorem fun\u00e7\u00f5es cognitivas como a percep\u00e7\u00e3o visual e o controle inibit\u00f3rio. O estudo transversal evidenciou que as fun\u00e7\u00f5es executivas, junto com o funcionamento motor, a percep\u00e7\u00e3o visual, os tra\u00e7os do espectro autista e o estresse parental, influenciam de forma importante na qualidade de vida. Fatores como o sexo, a idade, o n\u00edvel educativo, o diabetes tipo 2, a ades\u00e3o \u00e0 dieta mediterr\u00e2nea, o uso de metformina e o n\u00edvel de funcionalidade influenciam de maneira significativa na resposta cognitiva. Este trabalho revisou inicialmente a evid\u00eancia existente sobre o uso de novas tecnologias nesta popula\u00e7\u00e3o e, posteriormente, avaliou o impacto de um programa de treinamento cognitivo digital aplicado em Centros de Educa\u00e7\u00e3o Especial.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">Participaram crian\u00e7as e jovens de entre 6 e 17 anos, dos quais se recolheram dados sociodemogr\u00e1ficos, cl\u00ednicos, de h\u00e1bitos digitais e qualidade de vida, junto com avalia\u00e7\u00f5es comportamentais e neuropsicol\u00f3gicas mediante provas padronizadas. O programa de interven\u00e7\u00e3o realizou-se com a plataforma NeuronUP e constou de 24 sess\u00f5es de 20 minutos, aplicadas nos pr\u00f3prios centros educativos.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">Os resultados mostraram que as fun\u00e7\u00f5es executivas mais afetadas foram a inibi\u00e7\u00e3o, a flexibilidade cognitiva, o controle emocional e a autorregula\u00e7\u00e3o comportamental. Este estudo, realizado mediante tele-neuropsicologia em um ensaio cl\u00ednico randomizado e duplo-cego, avaliou o efeito de dois programas de interven\u00e7\u00e3o em 21 crian\u00e7as com diagn\u00f3stico de transtornos da aprendizagem: a abordagem Cognitive orientation to daily occupational performance (CO-OP) e a plataforma NeuronUP.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">Ambos os programas geraram melhoras moderadas na percep\u00e7\u00e3o do desempenho e na satisfa\u00e7\u00e3o em rela\u00e7\u00e3o com os objetivos acad\u00eamicos avaliados mediante a COPM. Este trabalho explorou o papel do exerc\u00edcio f\u00edsico e da atividade f\u00edsica na reabilita\u00e7\u00e3o cognitiva em DCA cr\u00f4nico, assim como a utilidade da telereabilita\u00e7\u00e3o mediante a plataforma NeuronUP.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">O primeiro estudo avaliou durante um ano a adultos com traumatismo cranioencef\u00e1lico severo em fase cr\u00f4nica que participaram em um programa supervisionado de exerc\u00edcio aer\u00f3bico. A interven\u00e7\u00e3o associou-se com um aumento da atividade f\u00edsica cotidiana. O segundo estudo analisou pacientes com AVC cr\u00f4nico durante a pandemia, combinando medi\u00e7\u00f5es de atividade f\u00edsica mediante acelerometria e 24 semanas de telereabilita\u00e7\u00e3o cognitiva com NeuronUP. Observaram-se associa\u00e7\u00f5es positivas entre atividade f\u00edsica e desempenho cognitivo, especialmente em pessoas menores de 60 anos, com padr\u00f5es diferenciais entre homens e mulheres. A ades\u00e3o \u00e0 telereabilita\u00e7\u00e3o foi elevada.<\/p>\n\n\n\n<p class=\"has-sm-font-size has-no-margin-bottom\">Ambos os estudos sugerem que promover a atividade f\u00edsica e reduzir o sedentarismo poderia potencializar os efeitos da reabilita\u00e7\u00e3o cognitiva em pessoas com DCA cr\u00f4nico, e que a telereabilita\u00e7\u00e3o, inclu\u00edda a realizada com NeuronUP, \u00e9 uma via promissora.
